Application Due April 28, 2022 — by 5 p.m.
Email completed application and scanned documents to:
donate@canyonsdistrict.org or drop off application to the Canyons
Education Foundation office at 9361 South 300 East

Note: my529 scholarship accounts will be owned by the Canyons Education Foundation in students name until they graduate from a Canyons District school. Funds can be used at any university, college or technical school. If the funds are not used within two years of graduation they will revert back to the Canyons Education
